Dear Parents:
The following information is for the 2011-12 school year.
Please note the NEW start and end times for the school day.
First Days:
- Grades 1-8 - Thursday, August 25, 8:20 a.m. - 11:45 a.m.
- Grades K-8 on Friday, August 26, 8:20 a.m. - 11:45 a.m.
- Early Childhood AM - Thursday, August 25, 8:20 a.m. to 11:00 a.m.
- Early Childhood PM - Friday, August 26, 8:20 a.m. to 11:00 a.m.
- Prekindergarten – Wednesday, August 31 (AM & PM sessions)
- Kindergarten Meet & Greet- Thursday, August 25, 10 – 11:30 a.m.
Full School Day:
Kindergarten through Grade 8
Beginning Monday, August 29, 2011
Hours for Central Park, Kolmar and Springfield Schools:
K-8 8:20 a.m. – 2:45 p.m.
AM PreK/EC 8:20 - 11:00 a.m.
PM PreK/EC 12:05 - 2:45 p.m.
Hours for Spaulding School:
K-3 8:25 a.m. – 2:50 p.m.
AM PreK 8:25 - 11:05 a.m.
PM PreK 12:10 - 2:50 p.m.
Registration:
August 11 and 12 at the school your child will attend.
Thursday, August 11 from 1 to 7 p.m.
Friday, August 12 from 9 a.m. to 2 p.m.
Residency:
Needed: Three (3) original proofs of verification of your residence at the address stated on the student enrollment form. (See Residency Requirements form.)
Student Information Form for all students. Please correct, add, and complete the form for each child and return on Registration Day. Extra forms will be available during registration.
Fees:
- Waiver and Lunch Applications: If fees are to be waived or lunch provided, proof of income, State lunch certificate, or qualifying SNAP or TANF number is needed.
- Prekindergarten: No Fee
Early Childhood: $35 per student
Grades K-8: $50 per student with a maximum of $100 per family.
Fee includes supplying Weekly Readers, workbooks, etc. - Activity Fee: $5 per child - PreK-6 activity fee will be used for three (3) parties during the school year, and thegrades 7 and 8 fee will be used for junior high activities. These must be paid for separately and in cash.
- Locker Fee: $5 fee for one combination lock at Springfield; $5 fee for one combination lock at Central Park and Kolmar (PE locker). Combination locks are rented through the school. All 7th and 8th graders at Central Park and Kolmar Schools and 6th graders at Springfield School MUST use school locks. Locks will be issued individually to the students. This fee is refundable in full at the end of each school year or upon transfer from the school when the student returns the lock, in good repair, recorded as issued to him/her.
- Milk Fee for each semester, if desired: $12 per student for low fat 2% white milk or 1% chocolate milk.
- Lunch - Purchased Lunches will be $2.00 per day, including milk. Information regarding eligibility for free lunches will be available at the schools during registration.
- School Accident Insurance: The District provides school-time coverage for students, but parents are encouraged to consider purchasing 24-hour coverage accident insurance at a nominal cost. If you wish to take advantage of this offer, it will be available at registration.

Supply Lists:
Specific Grade Level Information:
- Prekindergarten Students: If your child is accepted in the Prekindergarten Program, you will be notified.
- Kindergarten: A child must be five years old on or before September 1, of the current school year, to be eligible for enrollment in kindergarten. Medical, vision and dental forms, registration folder, and an official birth certificate must be submitted at registration.
- Sixth-grade Students: Updated medical/dental examination form* must be submitted at registration.
- New to District: Transfer students must present a transfer issued by the sending school. Medical/Vision/Dental * and an official birth certificate must be submitted at registration.
- Kindergarten, second, and sixth grade students: By May 15 of the current school year, proof of a dental examination by a licensed dentist is required.
Proof of Immunization:
Only for those parents who have received notices concerning proof of immunizations. Such proof must be received in order for your child to start school on August 26. This is required by law.
Medication:
If your child is taking medication which must be administered at school, please complete the appropriate form/s* and return it to your principal.

Other Forms:
- Sign-up Sheets for Volunteers: This is optional, of course, but we do hope many of you will be able to give us some time to assist with our many activities.
- Home Language Survey Form: We are asking parents new to the District to please complete and submit this form* at the time of registration.
- Bus Schedules will be available for Kolmar students only. (Kolmar 7th- and 8th-grade students who live south of 147th Street and north of the Midlothian Turnpike and live less than 1-1/2 miles from school, may purchase a bus pass, if seats are available, at $50 per semester.)
- Demographic Survey: The state requires each school to identify its school community in a variety of ways. The Demographic Survey will allow that requirement to be met. Only ONE (1) form per family per school is needed.
- Dangerous Weapons Policy Rules: This form* must be completed and on file for each child in the District. We ask that you read it carefully, discuss with your child(ren), sign, and return during registration (additional forms will be available during registration).
- Gym uniforms for grades 7-8: Parents will purchase a gym uniform (blue shorts and gray shirt) at a cost of $12.50 per student.

Kindergarten Meet & Greet
Parents are invited to attend an orientation with their kindergartners on Thursday, August 25th from 10 – 11:30 am. Students will be dismissed with their parents at the conclusion of the Meet & Greet session. Parents: Please make babysitting arrangements for younger children on this day. Meet & Greet is strictly for new kindergartners and their parents!
